I accept Premera Blue Cross insurance and can be billed as an out-of-network provider. In efforts to make my services more accessible, I offer a limited number of sliding scale slots. Please inquire during our initial consultation if interested in discussing this option.

Please confirm your insurance coverage and patient responsibility before your first appointment with me. Your co-pay or patient responsibility (deductible) determined by your insurer is due at each visit before your session begins.

I require 24-hour notice for missed sessions. Appointments canceled within the 24-hour window will incur my full fee.
